Step 1. Enter Your Parent/Guardian Information

The first page you’ll see says “Sign Up with Little Wellness Pediatrics.” The information on this page is for the parent or legal guardian. You’ll enter:

First name
Last name
Email address
Phone number
How you heard about Little Wellness Pediatrics

Don’t worry — you are enrolling yourself as a patient. Your information is simply entered first, and you’ll have the opportunity to add your child(ren) later in the enrollment process.

Step 2. Enter Your Address & Insurance Information

On the next page, you’ll enter your mailing address, along with information about any health insurance or healthcare cost-sharing coverage you have.

If you have traditional health insurance or a healthcare cost-sharing program, please select “Yes” when asked about coverage and begin typing to find your carrier.

If you have traditional health insurance:

Enter your Member ID when prompted. Please note that your Little Wellness Pediatrics membership fees and laboratory services will not be billed through your insurance.
We collect this information so we can make the most appropriate recommendations for your family based on your coverage, when applicable.
If you’re seeking reimbursement from your insurance company for eligible laboratory services, we’re happy to provide a Superbill for you to submit. Exclusions apply — please see your Membership Agreement for details.

If you have a healthcare cost-sharing program:

Many cost-sharing programs do not provide a traditional Member ID. If this applies to you, please enter 00000 in the Member ID field.

Step 3. Add Your Child(ren)

The next page is where you’ll add each child who will be enrolling as a member. For each child, you’ll enter:

Full name
Date of birth
Gender
Email address and phone number, if applicable

If your child is too young to have their own email address or phone number, you can leave those fields blank.

If you’re enrolling more than one child, look for “Add Another Member“ on the right-hand side of the page. Continue clicking it until you’ve added all of the children you’d like to enroll. Once all children have been added, click “Continue to Confirmation.”

If Your Baby Has Not Been Born Yet
You can still complete enrollment! Your membership will not begin, and you will not be charged, until your baby is born. When adding your baby under the dependent section:

For their name, enter “Baby” as the first name and your last name as the last name.
Enter 08/15/26 for their birthday.
For the medical questionnaire, enter “No” or “N/A” for the answers — except the question asking if your child is under 6 months. For that question, select “Yes.”
For the medical release form, enter “N/A” for all fields.

Once your baby is born, please let our office know so we can update their information and finalize everything for their care.

Step 4. Add Your Payment Method & Complete the Membership Agreements

On the confirmation page, you’ll add your payment method — this is the payment source your monthly membership subscription will be charged to.

Below your payment information, you’ll see several Membership Agreements that you’ll need to electronically sign or accept. Please read through each agreement carefully. Some are required for membership, while others are optional and will be clearly labeled as such.

Once you have reviewed and accepted all applicable agreements, click “Confirm.”

Step 5. Complete the Medical Questionnaire

After confirming your enrollment, you’ll be taken to a medical questionnaire. Please complete the questions for each child who will become a member of Little Wellness Pediatrics. Answer each question to the best of your ability — this information helps our team get to know your child and prepare for their care.

Step 6. Complete the Medical Release Form

After completing the questionnaire, you’ll be prompted to complete a Medical Release Form for each child. The information on this form should be for the provider or practice who is currently providing your child’s primary/general pediatric care. This allows our office to request the medical records we need for your child’s care.

If your child has been hospitalized or sees specialists in addition to their primary care provider, please contact our office. We may need to provide you with additional medical release forms for those providers.
